The Transitions Program is a case management program for youth who are transitioning from the support of the Peel Children’s Aid Society and who are marginalized from their families, school, community, and other support services. The program offers a community-based service, providing a consistent level of support to youth during this transition. The Transitions Program utilizes a strength-based, client-centred approach, focusing on immediate needs and long term goals. Additionally, the Transitions Program actively establishes and maintains partnerships with community services to increase understanding of the unique needs of these youth to ensure understanding and seamless support.
- Residents of Peel
- Youth between their 15th and 18th birthday, and their families
- Youth who are connected with Peel CAS, but are discontinuing involvement with Peel CAS and are in need of transitional support
- Youth who are disconnected from school and/or employment, and unable to effectively connect with and negotiate service from other agencies
- Youth with mental health issues and several of the following behaviours: substance misuse, aggression, criminal/anti-social behaviour, sexual acting out, chronic isolation from family, negative peer relationships
